Ossigenoterapia iperbarica (OTI) durante il periodo di emergenza per COVID-19

Il documento di posizionamento su “Ossigenoterapia iperbarica (OTI) durante il periodo di emergenza per COVID-19” (clicca per vedere allegato) è stato elaborato in maniera congiunta dalla Società Italiana Anestesia, Rianimazione e Terapia Intensiva – gruppo di lavoro per la ossigenoterapia iperbarica (SIAARTI), Società Italiana Medicina Subacquea e Iperbarica (SIMSI), Associazione Nazionale Centri Iperbarici Privati (ANCIP), Associazione Pazienti Trattati in Iperbarismo (AS.PA.T.I.).

La malattia respiratoria acuta da SARS-CoV-2 (COVID-19) sta impattando significativamente la nostra vita. 

Le disposizioni richieste dalle Autorità competenti sono stringenti ma necessarie per la riduzione del contagio: questo deve essere l’obbiettivo primario. Indipendentemente dalle rigide procedure di pulizia e disinfezione applicate, la terapia iperbarica viene erogata in camere multiposto ed espone alle criticità sia per la promiscuità dello spazio confinato che per la respirazione all’interno di maschere oro-facciali individuali.

Il documento riporta le linee di indirizzo proposte a tutti i referenti – medici invianti, operatori del settore, pazienti – dei Servizi di Ossigeno Terapia Iperbarica (OTI), pubblici e privati, presenti sul territorio nazionale per garantire la terapia iperbarica in ragionevole sicurezza anche nella inconsapevole presenza di soggetti portatori del virus SARS-COV-2 (è escluso il trattamento di pazienti con accertata positività al virus, eccetto per le emergenze con rischio per la vita.

Hyperbaric oxygen treatment (HBOT) for COVID-19 in severe patients is effective , works well!
Location: Hyperbaric Oxygen Center of Wuhan Changjiang Shipping General Hospital at Hubei.
During the containing COVID-19: Zhong Xiaoling'1 ,the director of the hospital's hyperbaric oxygen department, overcame all difficulties and started the first hyperbaric oxygen treatment for a critically ill patient with severe hypoxemia. During the treatment, patient’s hypoxic condition improved significar and the patient is much better than before according to the observation after therapy! The effect is remarkable!
This clinical practice initiated hyperbaric oxygen treatment(HBOT) of COVID-19!

Hyperbaric medical devices: SIMSI’s plan to promote the spread of hyperbaric technology

The Società Italiana di Medicina Subacquea ed Iperbarica (SIMSI) is actively commiitted ed to promoting the spread of hyperbaric oxygen therapy (HBO2). In Italy HBO2 is included in the Essential Levels of Care – the benefits guaranteed by the state to all citizens – using code 93.95, which has a value of € 81-120 per treatment. This is is for a scheduled 90-minute treatment for a seated non-critical patient, including medical or nursing assistance. The Italian market is managed by 50 hyperbaric centers that produce 150,000 HBO2 treatments per year, for an estimated value of around €20 million. In Italy the 10 main private companies in health care have not yet invested in hyperbaric medicine, as they are likely unaware of the potential (i.e., pro table/positive gross contribution margin). A “technology push” marketing approach may help with awareness. Regarding technology, Italian legislation currently exists only for multiplace hyperbaric chambers. SIMSI, through the Institute of Clinical Physiology (IFC) of the National Council for Research (CNR), has created a web portal as a tool that aims to facilitate understanding among manufacturers, operators and users.

INDICAZIONI DELL’OTI

L'OTI non è una panacea per tutti i mali, ma trova precisa e netta indicazione in tutte quelle patologie che riconoscono una diminuzione della tensione di Ossigeno a livello tissutale, insufficiente all'espletamento delle funzioni vitali, cioè sfruttando l'aumento delle pressioni e della quantità di Ossigeno disciolto nel plasma secondo leggi fisiche dei gas. La Società Italiana di Medicina Subacquea e Iperbarica (SIMSI) raccomanda che i quadri clinici in cui l'OTI non sia considerata di provata efficacia, o non siano ancora inseriti nelle indicazioni contenute nelle Linee Guida di riferimento, dovranno essere sempre preventivamente valutati per ogni specifico paziente in base al possibile beneficio rapportato al rischio e al costo del trattamento, nel rispetto della salute e della libertà dei pazienti, della buona pratica clinica nella nostra disciplina, del codice etico e deontologico della professione medica."
